Study finds limited value to procalcitonin in identifying BSI on admission

Among patients with suspected bloodstream infections (BSIs) who had blood cultures and a procalcitonin test on admission, the sensitivity of procalcitonin at a cutoff of 0.5 ng/mL was 68%, and empiric antibiotic therapy use was not meaningfully different with positive or negative results.

At a year, after-effects of COVID-19 hospitalization other than VTE similar to flu, sepsis

Compared to patients hospitalized with flu or sepsis, those with COVID-19 had increased risk of venous thromboembolism (VTE), stroke, depression, and anxiety in the 30 days after discharge, but over a year, only the risk for VTE was significantly elevated.

Continuous vs. intermittent meropenem administration did not improve outcomes in sepsis

A composite outcome of mortality and new antimicrobial resistance did not differ by whether critically ill patients received an equal dose of the beta-lactam antibiotic continuously or intermittently, an international randomized trial found.

Antibiotic regimens differ in associated risk of AKI in critically ill patients

Vancomycin and piperacillin/tazobactam were associated with a higher risk of acute kidney injury (AKI) than either vancomycin and cefepime or vancomycin and meropenem in ICU patients, particularly at longer durations and in those with normal kidney function on admission.

Septic shock outcomes better in patients who got hydrocortisone plus fludrocortisone

A retrospective analysis of patients hospitalized with septic shock and receiving norepinephrine found that 47.2% of those who began hydrocortisone and fludrocortisone simultaneously died in the hospital or were discharged to hospice, compared to 50.8% of those treated with hydrocortisone alone.
