Using an enzyme immunoassay after a positive nucleic acid amplification test was associated with a decrease in hospital-onset Clostridioides difficile infections and a reduction in related antibiotic use but no change in emergent colectomies.

Mandatory approval process for C. diff testing linked to lower hospital-onset infection rates
An intervention that required mandatory approval from an infectious diseases physician before testing for Clostridioides difficile was associated with a greater than 50% decrease in hospital-onset C. difficile infection rates at one hospital.

Stewardship program improved perioperative opioid prescribing
A project in a safety-net health system reduced use of hydrocodone-acetaminophen for surgical patients and increased prescribing of acetaminophen, ibuprofen, and oxycodone to them without increasing overall opioid use or returns to the ED.

Stewardship project in U.K. hospitals reduced average antibiotic dose duration at one year
The intervention included a decision aid that prompted clinicians prescribing antibiotics to classify inpatients' infection risk as possible or probable and then choose between stopping or continuing the prescription at 48 to 72 hours. They also received audit and feedback.

Opt-out protocol increased antibiotic discontinuations in suspected sepsis
Inpatients on broad-spectrum antibiotics despite negative blood cultures were screened with a 23-item safety check and then randomized to usual care or messages to their prescribing clinicians encouraging discontinuation of antibiotics.

Antibiotic stewardship strategies linked to lower antibiotic overuse at discharge
Hospitals that had review of antibiotics prior to discharge had significantly lower overuse of antibiotics at discharge in patients with community-acquired pneumonia or urinary tract infection, according to a recent survey of Michigan hospitals.
