Guidelines not always followed in patients with CAP

Antibiotic stewardship strategies linked to lower antibiotic overuse at discharge
Hospitals that had review of antibiotics prior to discharge had significantly lower overuse of antibiotics at discharge in patients with community-acquired pneumonia or urinary tract infection, according to a recent survey of Michigan hospitals.

Model for COVID-19 triage, updated guidelines for inpatients, negative trial of ivermectin
An electronic health record tool used nine variables to predict which inpatients with COVID-19 would deteriorate. Two societies updated their guidelines on inpatient thromboprophylaxis and remdesivir use. A trial found that ivermectin had no effect on disease progression or mortality.

Computerized clinical decision support may help reduce unnecessary C. diff testing
A two-year cohort study of nine academic and six community hospitals in the U.S. found that testing for Clostridioides difficile decreased when the electronic health record stopped a clinician from proceeding with a test order if the patient had already been tested or had recently received laxatives.

Microbiome therapeutic reduced risk of recurrent C. difficile infection in phase 3 trial
An industry-funded trial of patients with recurrent Clostridioides difficile infection who received standard-of-care treatment with antibiotics found that those who also received the investigational drug had a 68% lower risk of recurrence than those who received placebo.
