Patients discharged home from ICU had similar outcomes as those transferred to ward
A Canadian study found no difference in hospital readmission or ED visits within 30 days between patients who were discharged directly home after an ICU stay and those who were transferred to a ward before discharge.

Advance care planning assessed, therapeutic heparin debated in recent COVID-19 articles

Being visited in the ICU associated with lower risk for psychiatric outcomes
A retrospective cohort study found that over a third of critically ill patients were diagnosed with a psychiatric disorder in the year after discharge, but risk was lower in the 96% of patients who received in-person visits from family and friends during their ICU stay.

Real-time clinical decision support associated with improved pneumonia care
A trial at 16 hospitals in Utah found lower mortality rates, improved antibiotic prescribing, and increased discharge to home after deployment of guideline-based electronic clinical decision support for pneumonia care in the ED.

Benefits of dedicated COVID-19 hospitals, ECMO; air sampling after aerosol-generating procedures
A health system reported outcomes at two hospitals created to treat only COVID-19. Another found that 42.9% of COVID-19 patients who got ECMO died, compared to 89.1% when it wasn't available. A sampling study compared viral particles in the air with different procedures.

Guidelines not always followed in patients with CAP
A study of data from 177 U.S. hospitals found that almost all patients admitted with community-acquired pneumonia (CAP) received blood cultures and less than a quarter were tested for influenza.

The College's updated systematic review found that a 10-day course of remdesivir probably results in little to no mortality reduction but does probably moderately increase the proportion of patients recovered by day 29 and may shorten length of stay.

Metformin during hospitalization associated with improved outcomes in type 2 diabetes and sepsis
In a retrospective cohort study in Pennsylvania, adults with type 2 diabetes and sepsis who were exposed to metformin during hospitalization had lower 90-day mortality and better kidney outcomes than those who were not.
