Negative trials of prone positioning, antiplatelet therapy for COVID-19
Telling inpatients to lay on their stomachs did not significantly improve outcomes, one study found, while others offered some mixed results on the effects of aspirin and P2Y12 inhibitors. Additional research showed that a commonly used triage score doesn't work for COVID-19 and that hospitalists' worry about catching the virus impaired their well-being.

Advance care planning assessed, therapeutic heparin debated in recent COVID-19 articles
Along with an analysis of the pre-existing conditions of patients who died of COVID-19 and a point/counterpoint on use of heparin in ward patients, the latest inpatient COVID-19 research included negative trials of losartan and a potential monoclonal antibody.

DOACs better than heparin for periop thromboprophylaxis, review finds
The systematic review and network meta-analysis found lower rates of symptomatic venous thromboembolism in surgical patients given direct oral anticoagulants (DOACs) versus the standard dose of low-molecular-weight heparin, without a significant increase in bleeding.

Rule developed for starting DOACs after stroke in patients with atrial fibrillation
Using a “1-2-3-4-day rule” to choose when to start direct-acting oral anticoagulants (DOACs) based on stroke severity appeared to decrease risk for recurrent stroke or systemic embolism without increasing bleeding risk.

Adding P2Y12 inhibitor to heparin offered no benefit in non-critically ill COVID-19 inpatients
An open-label randomized trial at 60 hospitals in four countries found no increase in organ support-free days in patients who received a therapeutic dose of heparin plus a P2Y12 inhibitor versus heparin alone.

NIH guidelines on anticoagulation in COVID-19; research on statin use, omicron variant
The NIH updated its guidelines to address anticoagulation in COVID-19 inpatients, treatment with atorvastatin did not appear to improve outcomes in the ICU, and several studies looked at characteristics of omicron.

COVID-19 guideline update from NIH, postdischarge anticoagulation, remdesivir effects
The latest news on inpatient COVID-19 care includes some changes to the NIH guidelines for COVID-19 treatment, as well as studies showing that rivaroxaban after discharge reduced clots and remdesivir helped inpatients on no or low-flow oxygen but not those who were sicker.
