Extended anticoagulant treatment appears beneficial for first unprovoked PE
Eighteen months of warfarin after PE reduced recurrences during anticoagulation, but showed a similar thromboembolism rate as placebo at 42 months, a randomized trial found.

Review looks at optimal dosing for rapid reversal of vitamin K antagonists
A fixed-dose regimen of four-factor prothrombin complex concentrate was associated with lower mortality and fewer thromboembolic events compared with a variable-dose regimen, the systematic review and meta-analysis of anticoagulant reversal found.

Anticoagulation for heart failure and afib associated with reductions in mortality, but maybe not readmissions
Starting anticoagulation for older patients with heart failure and atrial fibrillation at hospital discharge was associated with improved mortality at 1 and 3 years but was not associated with improved rates of all-cause cardiovascular readmission or stroke, a study found.

Adding catheter-directed thrombolysis to anticoagulants not helpful in DVT
There isn't an apparent advantage to adding catheter-directed thrombolysis (CDT) to anticoagulants in treating deep vein thrombosis (DVT), a new study found.

Oral anticoagulants compared for risk of recurrent VTE, major bleeds
All oral anticoagulants and antiplatelet agents reduced recurrence of venous thromboembolism (VTE) compared with placebo, with aspirin reducing risk the least and vitamin K antagonists reducing it the most, a meta-analysis found.

Timing of stroke treatment, preventive therapies presented at stroke meeting
Mostly disappointing results on transfers for thrombectomy, delayed use of thrombolysis plus thrombectomy or tenecteplase, and anticoagulation for secondary prevention after cryptogenic stroke were reported at the International Stroke Conference.
