Diagnostic error found in care of 23% of patients who died or were transferred to ICU
The most common causes of diagnostic errors were assessment and testing issues, according to an analysis of patients who were transferred to an ICU, died, or both while hospitalized with general medical conditions at any of 29 academic centers in 2019.

Costs of care among Medicare patients were similar at teaching vs. nonteaching hospitals
